A CASE OF BILOBED GALLBLADDER

Abstract

Double gallbladder is a rare malformation in the presumed frequency of 0.02-0.03%, and especially its subtype of bilobed gallbladder has been scarcely reported. Recently we experienced a case of bilobed gallbladder with choledocholith which was successfully managed by laparoscopic cholecystectomy and choledocolithotomy with primary suture.
A 54-year-old woman was admitted to the hospital with a diagnosis of gallstone cholecystitis with pancreatitis. Preoperative examination revealed no choledocholith. Upon operation laparoscopic cholecystectomy was employed. Intraoperative cholangiography visualized a shadow defect in the common bile duct. The common bile duct was incised longitudinally by 8mm and the stone was removed by a Fogaty catheter, followed by choledocolithotomy with primary suture. The excised gallbladder had macroscopically normal morphology, but was bilobed gallbladder which was divided into two lobes by a septum in the lumen ranging from the fundus to the cervical portion. Gallbladder stones existed in both cystic spaces and was of mixed type. Histopathologically the both spaces were surrounded by proper fibromuscular tissue. Bilobed gallbladder presents difficulty in preoperative diagnosis, but it can be operated on by usual laparoscopic approach.
